    Earnest refinancing bonus

    I think the large amount of debt is a good motivator to get through medschool and residency.   But considering how competitive the students accepted into NYU medschool will be those particular students may not need any extra motivation.

    I saw something similar as a Medstudent. Emergency c section. The mom obviously could feel the c section and was in pain. Anesthesia was there from the start but couldn’t get the pt under fast enough and was yelling at the OB to slow down. But the Ob was not slowing down (for good reason – the baby died). It was Horrible. But I remember at the end of it the anesthesiologist said he was going to give the patient enough ketamine to make her forget the last couple hours of her life. I don’t know enough about ketamine — but is that possible?

    I wish I got paid like an ophthalmologist in Canada. They somehow make over twice as much as Ophtho does in the US. (This is true of many other specialties as well)

    But I think the key is that the Canadian govt saves money by negotiating drug prices. In the US, politicians get paid way too much by drug companies for that to happen, so the cuts will likely come from slashes to physician reimbursement and patient care.

    I thought Trumps administration and some judge recently killed the fiduciary rule…. and so “fiduciary” is now meaningless and you can’t legally be sure about any financial advisor.

    The zero down 30 year mortgage came out better with the prior tax laws for us due to mortgage interest deduction. I don’t think it still would and I don’t think these calculators are updated.

    Why do you think certain specialties (ER, anesthesia) form groups that negotiate contracts with hospitals while other groups (primary care) seem to be more frequently employed by hospitals individually? How do you think this affects reimbursement?

    Disclaimer: I’m Ophtho so I apologize if the above observation is completely wrong

    Also seems to increase the value of midlevels (or at least anyone who deals with the most straightforward patient complaints) relative to doctors.

    Sounds like more testing and procedures, less time talking to patients.

    I remember seeing news reports about Trump’s businesses going bankrupt throughout my childhood and I’d guess he would be wealthier if he just put his dad’s money into index funds. Then he refused to release any financial statements (tax returns) that would give any verifiable insight into his financial state but somehow still convinced people to elect him based on his supposed business credentials.
    Considering that, I don’t see how the general public will care at all about Kavanaugh’s finances

    A prior posting on this message board asked how we decide where to get our news from.  A news site that doesn’t name the supposed large medical organization that it’s writing the story about is getting crossed off my list.   I can understand not naming a minor or an innocent person,  but a large organization?   You can just start making up any news story you want then.   I and a number of other large organizations that-shall-not-be-named find this to be complete pointlessness.

    I’ve learned a lot from John Oliver. Despite it being comic it’s often very informative.

    His episode on retirement plans is pretty basic for all us now but I think we can all appreciate his efforts to inform the public:

    All news stories are the same because they all copy each other.  What reporter has time to actually find the news, when they can just copy it from another news site (or get contacted by someone with an agenda and get the news story fed to them) and generate just as much revenue from hits.

    I’ll try to go to the primary source, if possible.   So for example, recently the news stories were about what crazy things Donald Trump said in some rally in Montana.  So I’ll pull up a youtube video of his rally in Montana and, sure enough, he said some looney stuff.   If it’s a health article, clearly I’ll just go to the scientific journal.  For legal-political matters I’ll listen to a podcast by Preet Bharara (former US attorney for NY) and his guests.

    Use the phrase “no boxed gifts please” on your invitation. Depending on your cultures they will understand that to mean give checks or cash only as presents. That covered almost half the cost of the wedding for us

    What’s wrong with vanguard?

    Both US and international index funds should be fine in a taxable account. Broad index funds (like total stock market) are probably more tax efficient but if doing small cap vanguard has a “tax efficient” small cap fund.
